Nutritional Assessment: Conduct comprehensive nutrition assessments for all patients according to CMS guidelines and facility protocols.
Meal Planning & Counseling: Develop individualized nutrition plans that address protein needs, potassium/phosphorus restriction, and fluid management.
Lab Analysis: Monitor and interpret monthly lab results (e.g., Albumin, PTH, Phosphorus, Potassium) and adjust nutritional interventions accordingly.
Interdisciplinary Collaboration: Participate in monthly Patient Care Plan (PCP) meetings with other providers to provide holistic care.
Education: Provide ongoing education to patients and their families regarding the renal diet, mineral bone disease management, and the importance of phosphorus binders.
To be considered for this role, candidates must meet the following criteria:
Credentialing: Must be a Registered Dietitian (RD/RDN) with the Commission on Dietetic Registration (CDR).
Licensure: Must hold a current, valid Dietitian License in the state of Montana.
Experience: Must have a minimum of one year of professional work experience in clinical nutrition following your registration as a dietitian.
Strong communication skills to handle sensitive patient conversations.
Familiarity with Electronic Medical Records (EMR) and renal-specific software.
Board Certification as a Specialist in Renal Nutrition (CSR) is a plus, but not required.
