Services are to be performed in the hospital under Nursing Administration – Patient Coordination of the Navajo Area Indian Health Service within the Northern Navajo Medical Center.
Orientation will be 7 day and night shifts (4 days, 3 nights) before scheduling an independent shift
Hours will be rotating shift of days, evenings, nights, and weekends to include holidays.
Candidate must have a minimum of 2 years in the Tour Coordinator environment with at least 36 months total nursing experience.
Duties include:
- Perform patient care coordination duties to meet the standard for safe patient care delivery in the hospital (outpatient and inpatient) in accordance with technical direction provided by NNMC's Patient Care Coordinator Supervisor Nurse or designee.
- Collaborates with departments to coordinate patient care activities and support activities to ensure a smooth hospital operation during the assigned shift.
- Coordinates admissions, discharges, and transfers (intra-and, interfacility) of patients and documents disposition in shift reports. Shift reports are then sent to Nursing Administration and Executive Leadership.
- Coordinates callback for support departments, i.e. Facilities, Bio-Med, Laboratory, Radiology, Operating Room staff, Motor Vehicle Operators, and Medical Staff.
- Makes nursing rounds on the units during the shift to evaluate patient care needs, staffing, environmental, and safety aspects and calls in required resources if needed.
- Assists with patient care in emergency situations utilizing the nursing process and observes patients for conditions that may require emergency measures and exercises sound judgment in interpreting observations to the physician.
